Churchmere Medical Group is situated over two sites:

• Ellesmere Health Centre, Trimpley Street
• Clayton Health Centre, Whitchurch

The two sites cover a wide area focusing on both Ellesmere and Whitchurch towns and their surrounding areas.

How to register

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. You can register with us online (click the link below) or attend the surgery to collect a paper form to complete. This will be processed by the practice and we will be in contact with you if we require any further information.

Once you have been accepted as a patient, your medical records will be transferred to the practice. Your medical records may take several weeks to arrive at the practice. We advise all our new patients to make a routine health check a member of our team within the first three months of joining our practice.

Temporary registration

In limited circumstances you may be registered as a temporary patient for up to 14 days. Please speak to the one of our Patient Service Advisors for further information.
