Nurses
Our nursing team help with a wide range of patient care including practice nursing and minor illness/same day clinics. The team are involved with managing chronic diseases to health screening, for example, cytology (smear tests), to immunisations, including flu, pneumococcal, and those needed for babies as well as patient dressings and observations. We also have members of the team that see patients for minor illness or same day appointments. They work closely with GP colleagues to assess, treat, and provide advice to patients presenting with illnesses requiring same day treatment. Some of our nurses have also undertaken further qualifications to enable them to prescribe for our patients. The team carry out a wide range of roles allowing them to develop long-term relationships with individuals and families to better manage their conditions and improve patients’ physical and mental health and wellbeing.

Paramedics
Our paramedics work closely with GP colleagues at the practice as part of our multi-disciplinary team. They provide care to patients visiting the practice and also patients that are housebound or in other community settings. They are trained to assess, treat, manage or refer patients according to the nature and severity of their condition.

Medicines management
Made up of pharmacists, pharmacy technicians and administrators this team supports with the management of medicines at our practice and responding to medication queries from patients. The team helps by maximising benefits and minimising risks associated with medicines, as well as making the best use of resources allocated for medication. They take responsibility for some aspects of care management of patients with chronic diseases, including undertaking medication reviews to proactively manage patients and their care. They carry out a wide range of other tasks including medicines reconciliation after patients are discharged from hospital, reviewing and adding medicines requested in outpatient letters, monitoring of high-risk drugs, completing safety audits, responding to medication queries, processing prescription requests, liaising with community pharmacies and consenting patients for repeat dispensing. First Contact Mental Health Nurse
Our First Contact Mental Health Nurses look to offer support, advice and guidance around all areas of mental health. They will be able to provide brief intervention and self-help materials, discuss medication and refer to specialist Mental Health teams if required.

First Contact Physiotherapists
Our first contact practitioners are physiotherapists with enhanced skills. They can help patients with musculoskeletal issues such as back, neck and joint pain by assessing and diagnosing issues, giving expert advice on how best to manage conditions and referring onto specialist services if necessary. Patients with back and joint pain, including conditions such as arthritis, can now book an appointment at the practice with the first contact physiotherapist, rather than waiting to see a GP. By making it easier to access physiotherapist, patients will have quicker access to diagnosis and treatment, helping them to manage their conditions more effectively and recover faster, so they can get back to normal life quickly.
Healthcare Assistants
HCAs have an important role in general practice, providing valuable assistance to GPs and practice nurses. Our HCAs are trained to carry out care of patients independently, in line with their qualifications and competencies, with access to a Practice Nurse or GP for support when necessary. Our HCAs take blood, perform ECGs, blood pressure checks as well as giving vaccinations/injections, changing dressings, removing sutures and stitches and seeing patients for annual chronic disease reviews.

Nursing Associates
Nursing Associates are healthcare professional who work as part of a nursing team, providing direct patient care under the supervision of registered nurses. They bridge the gap between healthcare assistants and registered nurses, taking on more complex care duties than assistants while working alongside nurses to deliver high-quality patient care.

GP Assistants
Our GP Assistants support our GP’s in their day-to-day management of patients providing a support role, carrying out administrative tasks, combined some clinical duties such as ECGs, bloods and other observations. They contribute to the smooth running of appointments and improving patients experience in the surgery.

Community/Cancer Care Co-ordinators/Social Prescribing
We have a team of care coordinators available to help you via our Patient Services Team.
Patient Services Advisors
Our Patient Services Advisors play a crucial role in helping our patients get the right support at the right time. They are your first point of contact and committed to confidentiality. They may have to ask for additional information to be able to assist you. Debbie R – Debbie R is our clinical rota manager and is responsible for effectively managing our clinical appointment templates across all of our sites. Patient Services Advisors are part of the multi-disciplinary team and help to identify and signpost patients to available services, acting as a link between the patient and clinical team. Their aim is to direct patients to the best clinician for their condition which may not always be a GP. The team are also responsible for dealing with all incoming e-consultations and ensuring that they are directed to the most appropriate team within the practice. As well as dealing with incoming requests they also contact our patients to arrange follow-up appointments, repeat tests and pass on information from the clinical team. Administration
Our administration team handle all aspects of administration work. They deal with all incoming and outgoing correspondence, including scanning letters onto the patient record. They also summarise new patient records and process all referrals to secondary care.
